THE DOCTOR KNEW HE WAS GOING TO TRENZALORE EVER SINCE RIVER WHISPERED HIS NAME AT THE LIBRARY.
He knew he’d be put in a situation where he’d need to reveal his name. So he told River on the off-chance that she’d help him if the situation got dire.

THE DOCTOR’S INCARNATIONS DO NOT MATTER. MATT SMITH IS THE 11TH DOCTOR, BUT HE IS NOT THE ELEVENTH INCARNATION.
His incarnations aren’t related to how many regenerations he’s gone through
BUT HOW LONG IT’S BEEN SINCE HE’D GIVEN UP HIS ORIGINAL LIFE.
So people before you get angry at Moffat for ‘trying to rewrite the incarnations’, consider that point.
The Doctor’s incarnations are basically how long it’s been since John Hurt’s version of himself!
